I neglect everything but don't know why.even a very serious thing .my exams are 7 days to go but I'm taking it easy to face them I know I will fail but taking actions is impossible for me .when I plan to take actions I will get motivated but I'm getting distracted to take actions.I'm unable to recall what are my emotions and beliefs about this situation. Anyone tell me what to do to keep on track.

It's a lot easier to keep focus physically out in the 'world'  than in your head. Print pics or draw or write out what it is you want to stay focused on and tape it to the walls, etc. Also write why you want to focus on it. Get some emotion going behind it.  It sounds too easy to work, but try it. 

Prime yourself by youtubing Tony Robbins, or "motivation". Amazing how 10 minutes of listening to something like that can change our state of mind.
