What causes a bad trip?

i.e taking more than you can handle, but what else causes it? I don’t know if it’s as simple as my mindset and intentions going into the trip or if one of you guys have a better idea of this 

I feel wrong for posting this as if it’s common sense and just fear because of a larger realization but maybe someone just has tips
